On Lok is a non-profit organization that provides comprehensive healthcare services and support for frail elders and vulnerable adults in the San Francisco Bay Area. On Lok offers a service delivery model based on the Program of All-inclusive Care for the Elderly (PACE) that allows participants to receive integrated healthcare services at home and in the community, thereby avoiding the need for institutionalization. The organization was founded in 1971 by a group of Chinatown residents who were concerned about the mistreatment of elders in nursing homes. They established the first On Lok Senior Health Services center in San Francisco’s Chinatown to provide culturally-competent healthcare services to Chinese seniors. In 1986, On Lok received a grant from the Robert Wood Johnson Foundation to expand its PACE program, which led to the establishment of the first PACE program in the United States. Since then, On Lok has expanded its service area to include seven counties in the Bay Area, serving more than 8,000 participants annually. On Lok operates a network of centers that provide interdisciplinary healthcare services, rehabilitation, social support, transportation, and other services designed to help participants maintain their independence and quality of life. The organization also partners with local healthcare providers, including physicians, hospitals, and specialists, to ensure that participants receive the appropriate care and services they need.
